Every account needs a Placement Form.  Please click on link above to obtain Placement Form.

Please provide any and all information that will enable us to contact your customer.   Please send Statement of Account, Credit Applications, Personal Guaranty's, Phone numbers, Email Address and any Alternate Mailing Addresses  via PDF Format to:

Upon receipt we will immediately commence the Collection Process, should your customer contact you, we ask that you refer them back to Nationwide Services, Inc. to establish our credibility.
